  • Vivify Medical provides primary care and cardiology services in the Financial District of Manhattan, New York, NY. We focus on cardiovascular health, preventive care, and personalized medical treatment. Our board-certified physicians tailor care plans to each patient’s goals and risk factors.

Appointments & Visit Preparation

  • Depending on your symptoms and diagnostic needs, we may ask you to fast for 8 hours before testing and hold certain medications. Specific preparation instructions will be given when you schedule your visit.

    We offer support in English and Spanish. Just let us know your preference when booking.

  • Bring:

    • Insurance card and referral/prescription info (if applicable)

    • ID driver's license 

    • Copies of recent medical records, lab results, and imaging reports

    • Any patient forms you completed in advance

    This helps us understand your medical history and make accurate assessments.

  • Yes, we send appointment reminders via phone, text, or email a few days before your visit.

Administrative Policies

  • Please notify us 48 hours before extended visits or 24 hours before routine/ultrasound appointments if you need to reschedule. A $75 cancellation fee applies if we don’t receive proper notice; this fee is not covered by insurance and must be paid before scheduling future visits
