What Advanced Blood Testing Can Reveal About Metabolic and Cardiovascular Health

GLP-1 medications can lead to significant weight loss, but body weight alone is a limited measure of metabolic and cardiovascular health. Losing excess body fat can improve blood pressure, glucose regulation, and cardiovascular health. But weight loss alone does not show whether someone is maintaining muscle and strength or how their broader metabolic health is changing. Blood testing can add vital information about glucose regulation, insulin resistance, cholesterol and lipids, and other factors that contribute to cardiovascular risk. 

In our previous article, we discussed why muscle matters when using GLP-1 medications and why weight loss should not be evaluated by the scale alone. Blood testing provides another way to understand how the body is responding as weight changes.

Weight loss and metabolic health

Weight loss refers to a reduction in body weight. That weight can come from body fat, muscle, water, or a combination of these. Metabolic health is broader. It reflects how the body regulates blood sugar, responds to insulin, processes lipids, and how well these systems are functioning overall. 

Weight alone doesn't show how these systems are functioning. Two people can have the same weight and very different levels of metabolic and cardiovascular risk. Someone can also lose a significant amount of weight while still having insulin resistance, an unfavorable lipid profile, or other risk factors that deserve attention. 

Blood testing can provide additional information about these changes. Depending on the patient, this may include blood glucose and hemoglobin A1c, insulin levels when clinically appropriate, cholesterol and triglycerides, and measures of liver and kidney function. 

Muscle, insulin resistance, and blood sugar

Muscle plays an important role in metabolic health. Skeletal muscle is one of the body's most metabolically active tissues and a major site of glucose uptake. When the body becomes less responsive to insulin, it may need to produce more insulin to maintain normal blood glucose levels. This is known as insulin resistance. 

Insulin resistance can develop before routine blood glucose measurements become abnormal. Fasting glucose and hemoglobin A1c may remain within a normal range while the body is compensating to maintain those levels. Looking at glucose, A1c, and other relevant markers together can provide a better understanding of how the body is regulating blood sugar. 

Glucose reflects the amount of sugar circulating in the blood at a given time, while hemoglobin A1c provides an estimate of average blood glucose over the preceding several months. Insulin, when appropriate for the clinical situation, can provide additional information about how the body is responding to glucose. Triglycerides and HDL cholesterol can also contribute to the overall picture. Preserving muscle during intentional weight loss supports strength and physical function and helps the body continue to handle glucose effectively. 

Cholesterol, lipids, and cardiovascular risk

A standard lipid panel remains an important part of cardiovascular risk assessment. It typically includes total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides. These measurements can be influenced by many factors, including genetics, diet, body composition, insulin resistance, medications, and overall metabolic health. Looking at lipid levels alongside a patient's medical history and other risk factors can provide a clearer picture of cardiovascular risk. 

For some patients, more detailed lipid testing may provide additional information. Whether this is useful depends on the patient's history, existing risk factors, and what the clinician is trying to understand about their cardiovascular health. 

Weight loss to risk reduction

For someone taking a GLP-1 medication, weight loss can be an important part of improving metabolic health. What happens alongside that weight loss also matters. Changes in glucose regulation, insulin sensitivity, lipid levels, muscle, strength, blood pressure, and other cardiovascular risk factors can help provide a broader view of how health is changing. 

The appropriate blood tests will vary from person to person. A patient with a family history of cardiovascular disease may have different priorities than someone with insulin resistance, abnormal cholesterol, or other metabolic risk factors. Testing should be guided by the individual's medical history and risk profile. 

GLP-1 medications are one part of a broader approach to metabolic and cardiovascular health. Nutrition, resistance training, physical activity, sleep, cardiovascular fitness, and muscle preservation all contribute to long-term health.
